Developmental Pathways is seeking a Coordinator to join our Community Outreach department to coordinate support services for adults and/or children living in a variety of settings. Our Coordinator gets to communicate with a broad team of providers including professionals, direct care professional employees, families, and natural supports chosen by the individual or family being served. You’ll also get to assist with our department’s initiatives and outreach efforts.

This position is primarily based out of our Englewood, CO office, just south of the Denver Tech Center. Our Community Outreach team has implemented a mobile working environment to provide the team with more flexibility in their roles. We have two office locations in the Denver Metro area; one in Aurora, CO and an office in Englewood, CO. Travel between offices from time to time may be necessary - mile reimbursement is offered. We’re committed to a great work-life balance here, so full-time is 36-hour/week and our offices are open Monday - Thursday and are closed on Fridays. More about what you’ll get to do as our Coordinator!
  • Research, request, obtain potential services and/or providers to meet individual needs/ preferences as determined in the Family Support Plan (FSP) and/or Community Outreach Enrollment Form
  • Mentor individual’s service choices; help families determine priorities for service
  • Respond promptly and professionally to individual/family needs and requests
  • Develop and review FSP with family and multidisciplinary team where applicable
  • Prepare family for transitions to other services as appropriate
  • Attend and participate in meetings; schedule and facilitate as needed
  • Assist with initiatives, meeting outreach goals and with events such as Community Outreach Resource Fair, Holiday Giving Projects, Parent Night Out events
  • Complete/revise/update FSP for individuals enrolled in the Family Support Services Program
  • Complete Allocation Requests and Check Requests for individual/family needs in the Family Support Services Program and Community Outreach Program
  • Work with services providers and independent contractors to assure expenditures are within budgetary guidelines
  • Develop and maintain case files. Maintain COSMO notes and CCMS web data entry in a timely manner, as appropriate
Must haves to be our Coordinator!
  • Bachelors Degree in human services related field, preferred
    • OR 5 years with a combination of education and experience appropriate to the requirements of the position
  • Excellent oral and written communication skills; can effectively communicate with individuals served, families, staff and others
  • Working knowledge of the DD system, rules and regulations preferred
  • High degree of accuracy with time sensitive, detailed information
  • Personal vehicle in good operating condition for use during work as appropriate
  • Valid Colorado motor vehicle license and proof of motor vehicle insurance
  • Ability to meet and maintain our driving requirements; no suspensions and less than three (3) tickets in three years
