Job Summary
The Team Leader must have the skills necessary to direct, supervise and manage the Resident Services and hands-on activities of Certified Nurse's Aides, Personal Care Providers, and Qualified Medication Administration Person as well as the ability to perform the same duties as the CNA, PCP and QMAP. Essential Duties & Responsibilities (This list may not include all of the duties assigned and may be supplemented) 1.Supervise CNA, PCP, and QMAPs in regard to job performance, including counseling and disciplinary action, staff training, management of time sheets and staff evaluations, designation of duties, including developing and maintain the staffing plan for assigned areas and shifts, in conjunction with the Resident Services Director (RSD) 2.Responsible for ensuring proper facility procedures are followed during resident emergency situations and transfer procedures, including notification and documentation 3.Ensure and accountability for answering call lights promptly and proper documentation 4.Responsible for proper discharge of residents when applicable 5.Responsible for end of shift communication with care staff to perform daily documentation 6.Responsible for adequate shift to shift communication to ensure continuity of care 7.Participate in new admissions to ensure services are integrated on day of admission 8.Assist with the physical care of residents such as bathing, oral hygiene, dressing, undressing and other activities of daily living 9.Ensure implementation of daily assignments for care staff 10.Daily rounds and quality care monitoring 11.Administer medications and treatments per physician orders. Ensure that accurate and timely documentation occurs regarding medications in compliance with regulatory requirements, facility policies and standards of care 12.Rotate being "on-call" with other team leaders for care staff call ins and ensure that adequate coverage is obtained 13.Encourage socialization among residents; review and be familiar with resident chart as a tool to learn resident’s social history and interests and review with all caregivers, in conjunction with Resident Services Director and Activities Coordinator 14.Promote harmony and cooperation between shifts 15.Model core values and ensure implementation of quality standards 16.Responsible for the transfer and security of all facility equipment, including pager, radio, keys, etc, 17.Conduct staff meetings as needed to ensure all staff has accurate and timely information 18.Ensure staff maintains cleanliness of work areas and common areas on a daily basis 19.Communicate with nurses as needed 20.Assist in medication destruction as needed 21.Ensure procedures followed for proper notification of on-call administration 22.Responsible to conduct community tours when needed 23.In the absence of the RSD, you share in the responsibility for the overall conduct, well-being and safety of the community 24.Maintain confidentiality according to HIPAA standards 25.Other duties as assigned Skills / Requirements
Knowledge, Skills and Abilities:
1.Ability to perform essential duties. 2.Ability to understand and follow instructions. 3.Obey all safety rules Education/Certification/Experience 1.High school diploma or GED preferred. 2.Medication Certification required. 3.Assisted Care Certification, Food Handler's Certification, Valid Driver's License and current auto insurance preferred. 4.Experience in assisted living setting and multi-line phone system preferred but not required.
