The Mental Health Center Serving Boulder & Broomfield Counties is a private, non-profit corporation offering mental health to people of all ages -- children, teens, adults and the elderly.
Our Mission, To make available an integrated, comprehensive system of community-based mental health services to all people living in Boulder and Broomfield counties, with the priority of serving individuals and families affected by major mental illness and serious emotional disturbances. We are looking for a Health Coach to join our team! JOB DESCRIPTION: Provides direct support to clients in an integrated behavioral health/primary care program located in a community mental health center. Essential Duties: oManages data entry and tracking to support a disease management and stepped care approach. oActively provides case management support for wellness services and basic needs. oConducts psycho-educational groups and wellness classes in conjunction with other staff. oProvides education to staff regarding an integrated BHPC model. oProvides supportive counseling to clients regarding their targeted treatment action plans. oTracking laboratory data. oObtaining vital signs oWorking with prescribers to help monitor health and follow up of clients oAbility to obtain healthcare information from other healthcare providers oOutreach to FQHC and other healthcare providers oSchedule appointments with other FQHC’s oFormulate case notes to keep other providers informed of direct care oOther duties as assigned. Skills / Requirements
Medical Assistant Certification and prior experience preferred in direct client care in a behavioral health setting. Understanding of the integrated model and basic knowledge of physical & behavioral health diagnoses. Ability to work in variety of settings including hospital, homeless shelter, other community agencies, or jail. Knowledge of community resources and ability to make appropriate referrals. Self-directed and able to work independently using sound judgment. Ability to work well with a diverse group of individuals & to apply principles of cultural responsiveness. Strong team player, communication and interpersonal skills required. Excellent problem solving skills. Knowledge of MS Word and Familiarity with electronic health records. Must have reliable vehicle and willingness to travel from site to site. Must have current Colorado Driver’s License and a safe driving record. Minorities are encouraged to apply.
