Total Longterm Care is a non-profit PACE program that provides managed health care and supportive services designed to assist seniors who want an alternative to nursing home placement. With Total Longterm Care's assistance, participants can stay in their own homes and communities. The mission of Total Longterm Care is to enable frail individuals to live with dignity in their communities.
We are currently seeking a Home Care LPN to join our outstanding team! This is a float position. POSITION SUMMARY: Under the direction and supervision of the Home Care Services Coordinator administers medications and treatments, draws blood, provides participant and family teaching and supports those activities necessary to the preservation of organization in the home environment. Participates in the communication of the participant’s condition to Home Care Services Coordinator and multidisciplinary team. PHYSICAL DEMANDS AND WORKING CONDITIONS: 1. Physical Requirements - Requires full range of body motion including handling and lifting patients, manual and finger dexterity and eye-hand coordination. Requires standing, walking, pushing, bending, kneeling and reaching at arm’s length and overhead in clinic, rehab department and in community for prolonged periods of time. Must be able to transfer participants needing total assist. Must be able to make home visits to participants who live in non-handicapped accessible dwellings. Requires the ability to use and repair department equipment. Requires the ability to lift/carry up to 65 pounds and push or pull objects exceeding 150 pounds, using appropriate body mechanics. Requires intermittent driving throughout the day, between participant’s homes and the centers within the greater Denver Metropolitan area. 2. Visual, Hearing and Communication Requirements - Requires corrected vision and hearing to normal range, with or without reasonable accommodation. Must be able to document care provided in participants’ service records. Must be able to communicate verbally with participants their treatment plan, team members and participants caregivers. 3. Pressure Factor - Requires working under stressful conditions. Working conditions may be noisy and crowded and fluctuating indoor temperatures. Moderate pressure to meet scheduled appointments while dealing with frail and confused participants. Subject to participants that -may have the potential for verbal or physical aggression. 4. Environmental Conditions - May be exposed to a risk of bodily injury through contact with moving instrumentation, toxic substances, medicinal preparations, bodily fluids, communicable diseases and any other conditions common in a clinic environment. Subject to unpleasant odors and living conditions of participants’. May be subjected to adverse driving conditions including wet, icy, slushy and snow-packed roads. Skills / Requirements
QUALIFICATIONS:
MUST PASS A PRE-EMPLOYMENT PHYSICAL AND DRUG SCREEN. 1. Education - Graduate of an accredited Licensed Practical Nurse Program and licensed as a Professional in the State of Colorado. CPR certification. 2. Experience – One year of health care experience in hospital, long term or home health care setting. Minimum of one year prior experience working with a frail or elderly population required. 3. Skills/Knowledge - Knowledge of medical equipment and instruments. Knowledge of common safety hazards and precautions to establish a safe work environment. Skill in applying and modifying the principles, methods and techniques of practical nursing to provide on-going care for participants. Ability to perform phlebotomy functions. Skilled in identifying problems and recommending solutions. Skilled in preparing and maintaining records, writing reports, and responding to correspondence. Skilled in developing and maintaining department quality assurance. Skilled in establishing and maintaining effective working relationships with participants, medical staff, staff members and family caregivers. Ability to react calmly and effectively in emergency situations. Current Colorado Drivers License, reliable transportation, a satisfactory driving record and proof of auto insurance. Important Notes
